The address mentioned, 219 Carter street is the address listed for Charles L. Offutt in both the 1909 and 1911 Chattanooga City Directory. I can not explain why the date doesn't match up with the Tennessee Death Record that shows a June 1911 date.

"Completing thirty-four years of active service in the Chattanooga fire department, Capt. C. L. Offutt. of engine company No. 10, Alton Park, was voted retirement by the firemen and policemen's pension board last night on a full pension of $90 a month. Capt. Offutt's retirement will be as of July 15, it was announced by Sergt. Jo J. Ivins, secretary of the board. The fire captain joined the fire department early in 1906 and was promoted to a captaincy in March, 1917. He resides at 3707 Provence street. Capt. Offutt is one of the few remaining old-timers of the fire department, having seen the transition from horse-drawn to motorized equipment, and enlargement of the department through annexation of suburbs. The pension board meeting was attended by Cobb Hays, Sergt. Ivins. Detective P. R. Walker. Fire Marshal H. S. Elkins and Fireman L. M. McCormick."
