"The consequences of (?) (?) serious ill health I hereby tender my resignation as 2nd Lt. of Company C, Col. (?) (?) Reg't. (5th KY Calvary? (?) Division, to take effect immediately.
I hereby certify that I have no public property in my possession, that I have not been absent without leave, that I am not in debt to the Government and there are no charges against me which can effect my pay or any charges of any kind against me.
